Types of Dental Plans
All plans are administered by Delta Dental and have a large network of providers, include preventative and diagnostic benefits, and no pre-existing condition limitations. Other features such as additional cleanings per year for specific approved medically-based conditions and orthodontic benefits are covered in the plan documents. However, each plan has different features which should be carefully considered when trying to best meet your needs and the needs of your eligible dependents. For services exceeding $400, have your dentist submit a treatment plan to Delta Dental prior to receiving any services. This will assist you with managing your out-of-pocket costs.
Exclusive Panel Option (EPO)
- You must live in Colorado.
- In-network coverage only except in cases of an emergency.
- Claims are submitted by dental provider.
Preferred Provider Organization (PPO)
- In- and out-of-network coverage is available, including international coverage.
- You have a lower out-of-pocket cost when using an in-network (PPO) dental provider.
- You may be required to submit claims and file for reimbursement when using an out-of-network provider.
Direct Reimbursement
- You may choose any dentist, the frequency, and the nature of care.
- You pay entire cost of services then file for reimbursement.
- Cost is determined by dental provider.
Insurance Cards
- Delta Dental will provide insurance cards within approximately four weeks of processing your information, or you may print one from Delta Dental's website.
- Your insurance card will contain a unique subscriber (member) ID number which is not your Social Security Number. The subscriber ID number will be the same for all family members covered under your plan.
- Your subscriber ID number is 887-(Employee ID)-00. Your Employee ID number is the four-digit number you use when logging into Ceridian Time and Attendance.
- Until you receive your insurance card, you may have to pay for services in full and file for reimbursement. Contact your Delta Dental to find out their reimbursement procedure. Be sure to save all your receipts.
Termination of Coverage
If you are enrolled in one of the dental plans and you lose eligibility for any reason, contact HR for information on COBRA coverage.
