Dental Plan Options

If you select a medical plan, you must select a dental plan. At a minimum, you are required to enroll in employee-only coverage. Your dependent children may be added to your current plan by submitting the Benefits Enrollment/Change Form within 31 days of their third birthday, within 31 days of a qualifying life event, or during the next annual Open Enrollment period. Your dependent children under age three may be covered at no cost as long as you enroll them; you may then elect to ceased or continue their coverage once they turn three by submitting the Benefits Enrollment/Change Form within 31 days of their third birthday.

• In-network coverage available using Delta Dental PPO and Delta Dental Premier networks of providers.
• Preventive and diagnostic care not subject to the deductible.
• Orthodontic coverage only for children up to age 19.
