Important Information Concerning This Handbook and Your Employment

Your employee handbook contains valuable information and is important to your employment. Please read it carefully and use it as a reference throughout your employment with UPI. One of the first responsibilities is to become familiar with these contents.

THIS HANDBOOK IS DESIGNED TO ACQUAINT YOU WITH UNIVERSITY PHYSICIANS, INC. AND PROVIDE YOU WITH INFORMATION ABOUT SOME OF THE GUIDELINES, PROCEDURES, COMPENSATION AND BENEFITS WHICH APPLY TO YOUR EMPLOYMENT WITH UNIVERSITY PHYSICIANS, INC. THIS EDITION OF THE EMPLOYEE HANDBOOK REPLACES ALL PREVIOUS ISSUED EDITIONS.

** THE HANDBOOK IS NOT ALL INCLUSIVE, BUT IS INTENDED TO PROVIDE YOU WITH A SUMMARY OF SOME OF UPI ’S GUIDELINES.

ONLY EXPRESSLY AUTHORIZED OFFICERS MAY ALTER THE GUIDELINES OUTLINED IN THE EMPLOYEE HANDBOOK. IF ANY SPECIFIC PROVISIONS OF AN AUTHORIZED CONTRACT OR BENEFIT PLAN DOCUMENT CONFLICT WITH THIS HANDBOOK, THE CONTRACT OR PLAN DOCUMENT WILL CONTROL.

NO REPRESENTATIVE OF UNIVERSITY PHYSICIANS, INC., OTHER THAN THE CHIEF OPERATING OFFICER AND/OR EXECUTIVE DIRECTOR, HAS AUTHORITY TO ENTER INTO AN AGREEMENT OF EMPLOYMENT FOR ANY SPECIFIED PERIOD AND SUCH AGREEMENT MUST BE IN WRITING, SIGNED BY THE CHIEF OPERATING OFFICER AND/OR EXECUTIVE DIRECTOR AND THE EMPLOYEE.
